Detailed Description

This is the deposit for a diagnostic assessment with a Specialist Teacher holding an APC qualification.  This payment is in addition to the previously paid deposit.

The assessment report can be used as evidence in the future for support at school, university, and in the workplace depending on the current guidelines.  At the time of the final payment, you should return your parent/ carer and school questionnaires and confirm that an eye examination has been carried out in the proceeding 6 months.
