Ymunwch â ni mewn symposiwm i nodi diwedd astudiaeth a fu'n archwilio'r rhwystrau, y galluogwyr, a'r canlyniadau o ran sut mae pobl sydd wedi gadael gofal yn ymgysylltu â gwasanaethau amlasiantaethol (e.e. gofal cymdeithasol, tai, iechyd, addysg a'r trydydd sector).
Byddwn yn clywed gan ymchwilwyr, rhanddeiliaid allweddol ar draws meysydd gofal cymdeithasol, iechyd, addysg a thai, a chan bobl ifanc a fydd yn cyflwyno canfyddiadau ac allbynnau’r astudiaeth hon a ariannwyd gan Ymchwil Iechyd a Gofal Cymru.

Join us for an end of study symposium exploring the barriers, enablers, and outcomes to care-leavers engagement with multi- agency services (e.g., social care, housing, health, education and third sector).
We will hear from researchers, key stakeholders across social care, health, education and housing and young people who will present findings and outputs from this Health and Care Research Wales funded study.

Mewn cydweithrediad â phobl ifanc sydd wedi gadael gofal ac ymarferwyr, mae ymchwilwyr ym Mhrifysgol Bangor wedi datblygu model ymarfer wedi’i lywio gan dystiolaeth a phecyn cymorth i ymarferwyr sy'n gweithio gyda phobl sy'n gadael gofal. Gwerthuswyd hyn yn defnyddio dull

adenillion cymdeithasol ar fuddsoddiad i fesur effaith gymdeithasol y model ymarfer a’r pecyn cymorth.

 

Cynhyrchwyd cyfres o bodlediadau o’r enw 'Symud i fyny' / 'Moving up'   (https://www.listennotes.com/podcasts/moving-up-symud-i-fyny-bangor-university-t8qMISQ1Gbs/) yn canolbwyntio ar oedolion ifanc â phrofiad o fyw mewn gofal, ac ymarferwyr amlasiantaethol sy’n gweithio ar hyn o bryd yn cefnogi pobl ifanc ac oedolion ifanc ar draws system ofal Cymru. Caiff pennod olaf y gyfres o bodlediadau ei recordio yn ystod y digwyddiad hwn.

 

Bydd cyflwyniadau'r symposiwm yn cael eu cyflwyno'n ddwyieithog. Bydd gwasanaeth cyfieithu ar gael drwy gydol y dydd.

 

 

In collaboration with young people who are care-leavers and practitioners, researchers at Bangor University have developed an evidence informed practice model and toolkit for practitioners working with care-leavers. This was evaluated using a social return on investment to measure the

social impact of the practice model and toolkit.

 

A podcast series ‘Symud I fyny’ / ‘Moving up’   (https://www.listennotes.com/podcasts/moving-up-symud-i-fyny-bangor-university-t8qMISQ1Gbs/)  was produced focussing on young adults who have experienced care, and multi-agency practitioners currently working to support young people and young adults across the Welsh care system. The final episode in a series of podcasts will be recorded during this event.

 

The symposium presentations will be delivered bilingually. Translation will be available throughout the day.
